    Abstract: 現代人對自我健康管理的重視度日漸升高,也越來越多人投入開發基於個人健康紀錄(PHR)的相關系統。但大多系統都是以醫療專業人員使用為主,ㄧ般病患對其中的數據資料並不甚了解。所以,本文目的在提供一個以病人角度為主的個人健康紀錄系統架構。將醫療院所HIS紀錄中難懂的數據,透過HL7 XML格式的轉換,在資料庫中對照替換後,成為病患能了解的電子病歷紀錄,進而更積極參與個人醫療計畫。The modern people rises attention to self-health management day by day, and more and more people develop the system that based on personal health record (PHR). However, most systems are developed for the medical specialists to use, in other words the patinets hard to know the meaning of data/information in PHR system. In this paper we presented an architecture of patient’s view personal health record system. In this system we convert the data/information of HIS to HL7 format and compared with database of PHR system. Then, translated the meaning of data/information. Finally the electronic health record will become easier to read for the patients and then make the patients positively participates the individual medical plan.
